How to obtain licence?

Establish a documented Occupational Health and Safety Management Systems (OHSMS) and implementation plan and ensure its effectiveness by conducting minimum one internal audit and one management review.

Submit application on prescribed proforma (Form IV) , the questionnaire (Form X) in duplicate along with the quality manual and/ or concerned documents and necessary fees to Dy. Director General (of respective region).

Arrange for Stage I (Adequacy audit & Preliminary visit) and Stage II (Initial/Certification) audit.

Take corrective actions on non-conformities observed by assessment team and get them verified.

Make payment for required licence fee.
